    Rate yourself on a 1-10 scale in each of the areas below. 
    (1 is POOR, 10 is GREAT)
    Here we want to help keep track of what improvements are seen over time, especially in terms of reducing dosage or need. Always consult with your Doctor
    Write whatever notes, experiences or issues that you would like to.
